FEES

In order to make your visit to the Clock House Medical Practice as smooth as possible we have provided the following information on the financial arrangements which may apply to your visit. The aim of this information is to explain how your account is calculated, how it can be settled and payment methods available whether you are private medically insured or self funding patient.

Completing the registration form
Each time you are referred to a new Consultant or for each new condition we will ask you to complete a Clock House Medical Practice registration form. This registration form will remain valid for a period of 6 months.

This form asks for information necessary for the Clock House Medical Practice to process your account. If you are insured please complete the form in full including the name of your insurance company, policy or membership number, the date your symptoms were first noticed, the date you first consulted a GP, and a brief description of the initial symptoms. In addition if you are self funding please complete your credit/debit cards details.

For insured patients
If you have private medical insurance we ask that you contact your insurer prior to your visit to advise them that you are attending for an outpatient appointment and possible diagnostic tests. Your insurer will confirm if you are eligible for benefit and if any excess is payable. If you have not already contacted your insurer we recommend that you do so as soon as possible.

In order to speed up your claim please hand your completed claim form to the Consultant for signature at your initial consultation. You should then forward the form to your insurer.

Please let us know if you change address or insurance company.

For self funding patients
If you are paying for your own treatment and would like a guide on the price of any tests or procedures you may require please contact the Administration Manager who will be happy to help.

How you will be charged – for insured and self funding patients
If the Clock House Medical Practice has a direct settlement agreement with your insurance company any invoices relating to diagnostic tests, surgical procedures etc. will be sent directly to them. Your insurer will advise you if all or part of your claim will not be paid. If we have not received payment from your insurance company within 30 days we will send you the invoice to be settled. If you have any queries relating to an outstanding payment please contact your insurer directly. You will continue to receive correspondence from us if the account is not settled.

If your Consultant has a direct settlement agreement with your insurance company any invoices relating to consultation fees, tests or surgery undertaken will be also sent directly to them.

If you are a self funding patient or the Clock House Medical Practice and/or your Consultant does not have a direct settlement arrangement in place with your insurance company you will receive invoices for one or more of the following services:

Fees from your Consultant for: initial and follow up outpatient consultations, surgical procedure's undertaken in the theatre, procedures, tests and examinations undertaken in a consulting room or treatment room. You will receive an invoice directly from your Consultant. Please send a cheque made payable to the Consultant (not the Clock House Medical Practice), unfortunately payment cannot be made by credit/debit card for Consultant's fees. If you have any queries relating to your Consultant's account please contact their secretary.

Fees from the Clock House Medical Practice for: pathology tests, x-rays and ultrasound. If you have a surgical procedure in the theatre, or procedure/test/examination undertaken in a consulting room or treatment room you will also receive an invoice from the Clock House Medical Practice as well as from your Consultant. These fees are for the use of equipment, sterilising of equipment, nursing time, consumable items (dressings etc) and pharmaceuticals. We request payment within 7 days by credit or debit card (we accept most major cards) or cheque made payable to the Clock House Medical Practice. If you have any queries relating to your Clock House Medical Practice account please contact the Administration Manager.

Prescriptions
As part of your treatment your Consultant may provide you with a private prescription for drugs which you will need to take to a local pharmacy to be dispensed. The pharmacy will charge you as a private patient regardless of whether you would normally be exempt from paying for NHS prescriptions. Any queries relating to the cost of prescriptions should be directed to the pharmacy.
